How to distribute surveys via Klaviyo, send in-message surveys and NPS®

Survicate offers native, one-click integration with Klaviyo. It enables you to easily create surveys with Klaviyo-ready email templates. Once your customers start responding, you will be able to see your customers' data in Survicate. You can also send responses to mapped Klaviyo attributes or as tags. How to send Survicate surveys via Klaviyo

If you use a mailing list in Klaviyo to distribute your surveys, we automatically capture subscribers' first & last name and email address information - so you'll know who responded to your survey! You'll be able to filter survey results by these attributes, too. You can also add any attributes to your survey link that are supported by Klaviyo or connect Klaviyo integration.

There are two ways to distribute your survey with Klaviyo:

Step 1. Create your email survey

Use Create new survey button and select Email or shareable link survey. Pick whether you'd like to use a ready template, create one with the help of AI, or start from scratch. Now, build your survey by adding your questions, and applying survey logic.

If you want to embed your survey in an email, the first question of your survey needs to be either a Welcome message, Single choice, a Smiley scale, Rating scale or a Net Promoter Score. If you choose any other type of question as to the first one, we will only provide you with the link to your survey.

Step 2. Design your survey experience

Customize your survey page in the design tab, by adding your company logo, and changing colors. Make your surveys entirely branded as your own!

Step 3. Configure the survey settings

Once you are ready with your survey, head on to the Configure tab and pick Klaviyo as the distribution tool under 'Identify respondents with':

Under the Configure tab, you can enable other options like multiple responses.

Step 4. Set up integrations for your email survey

Enable Klaviyo integration or connect any other tool you use to send the responses collected to apps you're using.

Step 5. Share your survey

Head to the Share tab and decide how you'd like to send your survey.

  • Direct link

Grab a link to your survey from the Copy Link button, and hide it behind a CTA button or hyperlink.

  • Launch in an email

Display its first question directly in the email's body, and start the survey directly from the email. They'll then be redirected to a landing page to take the rest of the survey.

1. Click Get your code, then copy the HTML code.

2. Open your email builder in Klaviyo, go to Email Templates, and pick the template you need.

3. Choose the Text block where the survey should be placed or add a new Text block to the template.

Click on Source code, remove the text that you see there and paste the HTML code of your survey in the field.

4. Create a new Campaign with the use of your new template, send your email, and get ready to dive into your insights.


I've sent a test survey, but the response is anonymous. How can I know who answered the survey?

Please remember that sending a test email will make the responses anonymous. If you want to test the full experience as well as the integration, you need to send the email to an existing contact via the set live option.

Best practices

Send responses to Klaviyo as tags or attributes
You can enable Klaviyo integration in the Connect section of your survey and pass your survey responses back to Klaviyo user profiles.

Collect information about the respondents

When sending the survey via Klaviyo, we'll automatically recognize the respondent's first & last name and email. You can also add any other Klaviyo attributes to your survey link to identify the respondents with this data.
