If you use a mailing list in Klaviyo to distribute your surveys, we automatically capture subscribers' first & last name and email address information - so you'll know who responded to your survey! You'll be able to filter survey results by these attributes, too. You can also add any attributes to your survey link that are supported by Klaviyo or connect Klaviyo integration.

Use Create new survey button and select Email or shareable link survey. Pick whether you'd like to use a ready template, create one with the help of AI, or start from scratch. Now, build your survey by adding your questions, and applying survey logic.

Once you are ready with your survey, head on to the Configure tab and pick Klaviyo as the distribution tool under 'Identify respondents with':

Enable Klaviyo integration or connect any other tool you use to send the responses collected to apps you're using.

Head to the Share tab and decide how you'd like to send your survey.

Grab a link to your survey from the Copy Link button, and hide it behind a CTA button or hyperlink.

Display its first question directly in the email's body, and start the survey directly from the email. They'll then be redirected to a landing page to take the rest of the survey.

2. Open your email builder in Klaviyo, go to Email Templates, and pick the template you need.

3. Choose the Text block where the survey should be placed or add a new Text block to the template.

Click on Source code, remove the text that you see there and paste the HTML code of your survey in the field.

4. Create a new Campaign with the use of your new template, send your email, and get ready to dive into your insights.

I've sent a test survey, but the response is anonymous. How can I know who answered the survey?

Please remember that sending a test email will make the responses anonymous. If you want to test the full experience as well as the integration, you need to send the email to an existing contact via the set live option.

Send responses to Klaviyo as tags or attributes
You can enable Klaviyo integration in the Connect section of your survey and pass your survey responses back to Klaviyo user profiles.

When sending the survey via Klaviyo, we'll automatically recognize the respondent's first & last name and email. You can also add any other Klaviyo attributes to your survey link to identify the respondents with this data.
